To request an appointment, please complete our euthanasia request and release form. Dr. Smith will respond with available dates/times for an appointment within 12 hours of receiving your release form. Her fee for services is a flat rate of $450, this does not include cremations service fees.For all emergencies, please contact your nearest emergency veterinary clinic immediately.
